DATA REBATE Consumer Service Center

What are the steps to completing a rebate submission?

Step 1: Read the rebate form carefully.

Step 2: Gather the materials specified on the rebate form. Most rebate programs require:
. A receipt showing the purchase date and the item(s) purchased
. A UPC from the product packaging

Some rebate programs also require additional information such as a serial number, or activation code. These additional requirements will be included on the rebate form. Take great care in including all of the items indicated on your rebate form.

Step 3: Complete the rebate form in neat handwriting. Alternatively, you can use a preprinted mailing label.

Step 4: Provide an email address for up to the minute processing updates. Your email information is protected under our privacy policy.

Step 5: Make photocopies of all materials you are submitting for the rebate. Keep your copies handy as we will need them should a problem arise with your rebate submission.

Step 6: If you are mailing the submission, place the materials in an envelope addressed to the address indicated on the rebate form. Include a return address on the envelope so that the materials will be returned to you if the envelope becomes damaged or misdirected before it arrives at the mailing address.

Step 7: If you are mailing your submission, drop it off at any post office or mailbox prior to the postmark date on the rebate form. If you are faxing your submission, fax your submission prior to the postmark date on the rebate form.

Why should I make photocopies of my materials?

Submissions can be damaged during mailing which hinders our ability to properly process your rebate. Making photocopies of your rebate materials provides you with a safeguard- you can submit the copies by mail fax or email and we can use this to fill in any missing or incomplete information in your rebate record.

Where do I find the original UPC to submit with my rebate request?

The original UPC, the product bar code consisting of 12 numbers, is usually found on the bottom, side or inside flap of the box and looks like the example below. The numbers vary for each UPC, but the basic pattern remains the same.one number on the outer left, 10 numbers in the middle, one number on the outer right hand side. A UPC will NEVER contain letters.

This is an example UPC.

What is the serial number?

The serial number is a series of numbers (and sometimes letters) that can be located on the product itself. The serial number is often located on a small sticker, metal plate or other label affixed to the back or bottom of the product. The serial number is different from the UPC, which is located on the product package.

If your rebate requires a serial number, please be sure to include it with your rebate submission. If you are not sure where the serial number is located, check the product's manual, contact the rebate sponsor, or contact us at 1-877-630-2345.

What do you mean by the "purchase date" and "postmark date"?

The "purchase date" is the actual date on which the purchase was made, as printed on your receipt. The purchase date must be during the specified date range on your rebate coupon. The "postmark date" is the date on which the post office processes and stamps your envelope. You will only qualify for a rebate if your envelope is processed by the post office on or before the specified postmark date.

Can I send you a copy of my receipt and keep the original?

Absolutely. You can send a copy of your receipt, unless otherwise specified on the rebate coupon.

How can I apply for a rebate if I lost the original receipt?

To give you a rebate, we need a copy of the purchase receipt. Most stores can reprint a receipt for you. Please contact the retail store, distributor or website where you purchased the product.

If there is a rebate on a product and I bought more than one, can I claim a rebate for each one?

Possibly. See the rules on your rebate coupon.

I purchased a product online and did not receive a receipt. Can I still request a rebate?

Virtually all purchases made online usually do include an e-mail confirmation, which includes the product information, the purchase price and the purchase date. For these purchases, the e-mail confirmation may be used as your receipt. Contact the store if you did not receive it.

I forgot to send one of the required items with my rebate submission. What should I do?

Hold on to the item. We encourage you to resubmit your claim with the correct information within 30 days from the date of this communication. Be sure to include your name, address, phone number, promotion name and tracking number.

How long does it take to receive my rebate?

It's been longer than 8 weeks and I still have not received my rebate. What should I do?
